കുട്ടിയുടെ ജനനം ഏത് തദ്ദേശ ഭരണ സ്ഥാപനത്തിന്റെ പരിധിയിലാണോ നടന്നത് ആ തദ്ദേശ ഭരണ സ്ഥാപനത്തിലാണ് പേര് മാറ്റാൻ അപേക്ഷ നൽകേണ്ടത്. ആലപ്പുഴ മുനിസിപ്പാലിറ്റി മുഖേന അപേക്ഷിക്കാൻ കഴിയുകയില്ല.
