I have not included my child’s name in Birth Certificate. What is the procedure for Name Inclusion in Birth Certificate in Bangalore?

If the applicant is below 6 years old, please follow the below procedure

  1. Get Birth Application & Name inclusion Form from BBMP Office which is available  at Free-of-cost
  2. Fill the required details and get it signed by applicant’s parents.
  3. Hospital Discharge Copy OR Birth Certificate, if previously obtained
  4. Photo ID of Applicant
  5. Submit the form along with the required documents at BBMP office

If the applicant is above 6 years old, please follow the below procedure

  1. Get Birth Application & Name inclusion Form from BBMP Office which is available  at Free-of-cost
  2. Study Certificate from School which contains Child's name, Father's name & Mother's name and Date of Birth (or) Any educational document which contains Child's name, Parents' Name and Date of Birth
  3. Hospital Discharge Copy /previously obtained Birth Certificate
  4. Submit the form along with the required documents at BBMP office
