1 . Medical certificate issued by district medical officer in prescribed form

2 . Proposal send through proper channel

3 . If any difference in percentage of disability in i d card and in medical certificate it should be countersigned by district medical officer

4 . Physical handicapped ID card issued by district medical officer

5 . Terms and conditions of govt order fd 01 srp 79 dt 15.4.1980 and fd 31 srp 2007 (1) dt 14.8.2008 fd12 srp 2012(x)dt 16.2.2013 are applied
