If you don’t have a birth certificate or details in birth certificate is incorrect. In such a situation, It is very important to execute an Affidavit of birth with Notary public attested

An affidavit of birth can be executed by your parents or relatives in India. The following is the step by step procedure to execute an affidavit of birth in India.

Step 1: Prepare the draft in word format (We added a mock draft at bottom of this answer for your consideration)

Step 2: Buy Rs. 100 e-stamp paper from nearest co-operative bank, sub-register office, court, or independent stamp vendor.

Step 3: Print the draft on e-stamp paper, use normal A4 size paper for consecutive pages. Just one side print, no back to back print.

Step 4: Your parents or relative should carry a printed affidavit and supporting documents. Meet notary public nearby.

Step 5: Notary public reads affidavit, cross verify supporting documents. Your parents or relatives should sign an affidavit in front of notary public. Notary seal and sign the affidavit. Notary makes an entry in notary book.

This completes the execution of an affidavit for a birth certificate.
