Try applying with you name as on Aadhaar card and submit the aadhaar registered mobile number

Try using IE and redo the option of Application→Upload & e-sign documents, then it will go through

I worked it around with two means - either click on Advanced and then click on proceed. That should solve. Besides, it is advised that you use Firefox with respect to BBMP Sakala.
