You have to submit the duplicate PAN Card with your Jurisdictional Assessing Officer (Contact Income Tax Office in your locality/District) along with a letter that you were alloted another PAN Card by mistake. Supporting docs to be submitted: Copies of both PAN Card alloted and Aadhar Card as ID & Address proof.

You may fill and submit PAN Change Request application form by mentioning the PAN which you are using currently on top of the form. All other PAN/s inadvertently allotted to you should be mentioned at item no. 11 of the form and the corresponding PAN card copy/s should be submitted for cancellation along with the form.

The Govt. fees is 107/- INR but i suggest you to contact any agent/ CA who would fill this form around 200/- INR.
