Form 2 പൂരിപ്പിച്ചു RC Bookമായി അടുത്തുള്ള കേരള മോട്ടോർ ട്രാൻസ്പോർട്ട് വർക്കേഴ്സ് വെൽഫെയർ ഫണ്ട് ബോർഡിൽ സമർപ്പിക്കുക.

പഴയ വണ്ടിയുടെ ഡ്രൈവർ ആണേൽ Form 3 പൂരിപ്പിച്ചു ഐഡന്റിറ്റി കാർഡുമായി സമർപ്പിക്കുക.

പുതിയ ഡ്രൈവർ ആണേൽ Form 1 പൂരിപ്പിച്ചു ഡ്രൈവറുടെ ഐഡന്റിറ്റി കാർഡും,റേഷൻ കാർഡും,ആധാർ കാർഡും,ഡ്രൈവിംഗ് ലൈസൻസും, 2 പാസ്പോര്ട്ട് സൈസ് ഫോട്ടോയും സമർപ്പിക്കുക.
