Documents Required for Birth Certificate in Kerala

Following documents are required for the Birth Certificate in Kerala.

  • Application is to be made in the prescribed format

  • If the birth takes place at a dispensary/hospital, the registered record

  • Address proof(Voter ID, Driving License, etc.) of parents

  • Aadhaar Card of Parents

  • If the birth takes place at a house, the citizen has to approach the local body and inform in writing. Then it will be verified & the certificate will be issued

  • Certificate of Doctor / Hospital discharge certificate

  • In case of delayed registration, an affidavit from judicial magistrate / Notary is required indicating the date and place of birth, names of parent and evidence of the event of birth

  • In case you have applied after a month but within a year you have to obtain permission from the Add. Dist.Registrar i.e BDO of concerned Taluk

  • In case applied after one year, you need to obtain an order from the executive magistrate and a copy of the said order is required to be attached with the prescribed application form

  • If there is no record for birth/death from the hospital, then the village head / regional councillor / MLA / MP / MBS Certificate of signature and stamp with any one of the doctors should be submitted

  • If a child is born outside India and the parents are Indian citizens, then the event can be registered at the Indian Consulate under the Citizenship Act, 1955, and that birth need not be registered again under the Registration of Births and Deaths Act, 1969 in India.

How to Register Birth in Kerala?

Follow the below steps to register birth in Kerala.

  • Visit Sevana website.

  • Click on 'Online form submission'.

  • Select the District, Local body type and Local body.

  • Click on 'Submit'.

  • Select the Request type as "Birth Registration".
  • If you are an existing user login using the User name and Password. If you are a new user, select "New login".

  • Click on New Application Submission.

  • Fill the details of child such as Date of Birth, Gender, Name, Parents name, address etc. 

  • Registration of birth can be done without the name of the child in Kerala. A provision has been made in the State Rules enabling the Register to enter the name of the child in the register in respect of the birth already registered by him after getting the information from the parent or guardian of such child within one year from the date of registration. The name of the child can also be entered even after one year on payment of the prescribed late fee as per the provisions made in the State Rules.

  • Select the Request type as "Name Inclusion (Birth)".
  • If you are an existing user login using the User name and Password. If you are a new user, select "New login".

  • Click on New Application Submission.

  • Enter the name of Mother and Father.

Birth Registration Procedure in Kerala

The time for notifying the occurrence of the birth, death or stillbirth event to the Registrar is 21 days from the date of birth. 

In cases where the report is received within 21 days of birth

Birth must be reported to the local authority within 21 days of birth for registration. No fee shall be charged for applications so notified. In case of e-file, data entry should be done and necessary attachments should be included. If all the information is saved within 21 days, the data will be downloaded in the respective local government bodies. There is no application fee for this type of normal registration. 

Below are the items to be attached.

  • Birth Report Form No. 1 (Birth) should be scanned and attached

Documents Required for Delayed Birth Registration in Kerala

In case of reporting 21 days to 30 days after birth

Delayed Registration is in Category 1, if it is 21 days to 30 days after birth. A late fee of Rs.2 will be charged.

Below are the items to be attached.

  • Birth Report Form No. 1 (Birth)

  • Scanned copy of apology letter showing reason for late reporting of birth with court fee of Rs.5

In case of reporting 30 days days after birth to 1 year

Delayed registration is in category 2 if it is 30 days to one year after birth. A late fee of Rs.5 will be charged. In Gram Panchayats District Registrar (Deputy Registrar) should get permission from Panchayat. In Municipalities, Municipal Secretaries should give permission)

Below are the items to be attached.

  • Birth Report Form No. 1 (Birth).

  • Application for permission showing reason for delay in registration of birth (Court fee of Rs.5 to be affixed) Scanned copy of this application.

  • An affidavit (scanned copy) certified by a notary public or gazetted officer in state government service that no such birth has been registered.

  • Scanned copy of non-availability certificate from the local authority stating that this birth has not been registered before. Get Non-Availability Certificate from the Registrar's office. (The Non-Availability Certificate is an acknowledgement or endorsement from the authorities stating that the certificate is not available with them. Applicants have to fill out a form and submit it to the Registrar, who will then verify the data and issue the acknowledgement)

In cases after one year of birth

Delayed registration after one year of birth is in category 3. A late fee of Rs.10 will be charged. Registration can be done only with the permission of Sub Divisional Magistrate.

Below are the items to be attached.

  • Birth Report Form No. 1 (Birth)

  • Scanned copy of apology letter showing reason for late reporting of birth with court fee of Rs.5

  • Scanned copy of non-availability certificate from the local authority that this birth has not been registered before.

How to Register Birth Occured Abroad in Kerala?

Registration of Births Abroad is required only if the parents are coming with the intention of settling in India. The parents should register with the local authority of the place where they intend to settle.

The child should be registered within 60 days of arrival in India.

Documents to be submitted (Attachment)

  • Intending to settle permanently in India (declaration on Rs. 50 stamp paper.

  • Birth Report Form 1

  • Copy of passport of parents.

  • Any document showing the child's date of birth (mother's or child's passport hospital record/any other document)

  • Copy of flight ticket from abroad to India. No registration fee.

For applications submitted after 60 days of the child's arrival in India and within one year of the child's birth, the late fee is Rs 5

  • Declaration of intention to settle permanently in India on a stamp of 50 rupees.

  • Birth Report Form 1

  • Copy of passport of parents.

  • Any document showing the child's date of birth (mother's or child's passport hospital record or any other document).

  • Scanned copy of Affidavit showing reason for delay attested by Notary Gested Officer.

  • Copy of flight ticket from abroad to India.

Late fee is Rs 10 for applications submitted after 60 days of the child's arrival in India and after one year of the child's birth.

  • Declaration of intention to settle permanently in India on a stamp of 50 rupees.

  • Birth Report Form 1

  • Copy of passport of parents.

  • Any document showing the child's date of birth (mother's or child's passport hospital record or any other document).

  • Scanned copy of Affidavit showing reason for delay attested by Notary/Guest Officer.

  • Copy of flight ticket from abroad to India

How to Correct Birth Certificate in Kerala?

Corrections can happen in Child's name, Parent's name (Minor Corrections, such as spelling mistakes, surname inclusion, initials inclusion), Address, Hospital Name or total name correction of Parents which alters the main name.

Follow the below procedure for changing the name in the Birth Certificate in Kerala.

a) Correction In Child's name.

  • Request letter from the parent whose child's name has to be corrected

  • Photo ID of Parents

  • Joint Affidavit of Parents

  • Educational document of the child whose name has to be corrected, if any
