മരണ രജിസ്റ്ററിൽ പേര് തിരുത്തൽ വരുത്തുന്നതിന് ചുവടെ പറയുന്ന രേഖകൾ അടങ്ങിയ അപേക്ഷ ബന്ധപ്പെട്ട രജിസ്ട്രാർക്ക് നൽകണം.

1) വില്ലേജ് ഓഫീസറുടെ പക്കൽ നിന്നുള്ള വൺ ആൻഡ് ദ സെയിം സർട്ടിഫിക്കറ്റ് 

2) സ്കൂൾ രേഖ, ഡ്രൈവിങ്ങ് ലൈസൻസ്, പാസ്പോർട്ട്, EPIC, റേഷൻ കാർഡ്, ആധാർകാർഡ് എന്നിവയിൽ ഏതിന്റെയെങ്കിലും പകർപ്പ് 

3) മുൻപ് കൈപ്പറ്റിയിട്ടുള്ള മരണ സർട്ടിഫിക്കറ്റുകൾ.

4) മുൻപ് കൈപ്പറ്റിയിട്ടുള്ള മരണ സർട്ടിഫിക്കറ്റുകൾ നഷ്ടപെട്ടിട്ടുണ്ടെങ്കിൽ അതുസംബന്ധിച്ച 200 രൂപ മുദ്രപത്രത്തിലുള്ള സത്യവാങ്മൂലം. (നോട്ടറി/ഗസറ്റഡ് ഉദ്യോഗസ്ഥൻ സാക്ഷ്യപെടുത്തിയത്.) 

5) ആശുപത്രിയിൽ നിന്നാണ് മരണം റിപ്പോർട്ട് ചെയ്തിട്ടുള്ളതെങ്കിൽ ആശുപത്രി അധികൃതരുടെ തിരുത്തൽ കത്ത്.
