Whom to approach for this service : (Designated Officer)? DCT/Sr.RTO/RTO/ARTO

Procedure involved to get this service : a)applicant shall apply in form 29 of CMVR in duplicate ,application form 30 of CMVR along with documents before licensing authority b)submit the application at inward section for acknowledgement c)half of the registration fee referred in SL.NO 4 of rule 81 of CMVR and smart card fee Rs 200

Who are eligible to get this service : any Indian person above 18 year age

Documents to be enclosed with the request :

1 .  APPLICATION FORM 29 OF CMVR IN DUPLICATE ,APPLICATION FORM 30 OF CMVR ALONG WITH ADDRESS PROOF AND RECENT 3 PHOTOS

2 .  ORIGINAL REGISTRATION CERTIFICATE, COPY OF VALID INSURANCE CERTIFICATE,VALID TAX CARD AND VALID POLLUTION UNDER CONTROL CERTIFICATE

3 .  ORIGINAL CLEARANCE CERTIFICATE (CC) IN CASE OF VEHICLE MIGRATED FROM OTHER REGION EXCEPT MOTOR CYCLE

4 .  PAN CARD OR FORM 60 IN CASE OF LMVS AND TRANSPORT VEHICLE

5 .  VALID FITNESS CERTIFICATE IN CASE OF TRANSPORT VEHICLE

6 .  SELF ADDRESSED STAMPED SPEEDPOST ENVELOPE

Fee/Charges to be paid to get the service half of the registration fee referred in SL.NO 4 of rule 81 of CMVR and smart card fee Rs 200 and for delay after 14 days Rs 25 per calendar month or part thereof provided that amount shall not exceed Rs 100

Maximum number of days to wait to get this service delivered : 21 Working Days

Whom to approach as a appeal (Competent Officer), if the service is not delivered in time or rejected by officer : Joint Commissioner for Transport (JCT)

Maximum number of days to wait to get the decision of the Competent Officer : 30 Working Days

Whom to approach as 2nd appeal (Appellate Authority), if the decision of the Competent officer is not acceptable or not implemented? : Commissioner for Transport(C.T)

Maximum no. of days to wait to get the decision of Appellate Authority : 30 Working Days
