Whether it is necessary to submit forms in RTO, like Form 28, Form-29 mandatory for claiming insurance for the lost vehicle?

Form 28 is for Grant of NOC. If you have applied for Vehicle Re-registration or Ownership transfer in the past, then the insurance authorities might ask you to submit Form 28. For example, if you bought your vehicle from Delhi and have re-registered in Karnataka at the time of the theft, then the insurance authorities might ask you to submit Form 28.

Form 29 is for ownership transfer. If you have bought a second-hand car, then you might have got the ownership transferred in Form 29.

So if you don't belong to any of these categories, you don't need to submit Form 28 or 29.
