Procedure involved to get this service : a) Physically challenged should submit the prescribed forms along with the required proof of documents

Form to be submitted to get this service : Prescribed form / written application

Who are eligible to get this service : Physically challenged

Documents to be enclosed with the request :

 1 .  1. DULY FILLED IN PRESCRIBED APPLICATION FORM OR WRITTEN APPLICATION.

2 .  2. DISABILITY CERTIFICATE ISSUED BY DISTRICT MEDICAL BOARD OR OFFICER CERTIFYING THE EXISTENCE OF ABOVE 70 PERCENT DISABILITY.

3 .  3. PASSPORT SIZE TWO PHOTOS.

4 .  4. ADDRESS PROOF.

5 .  5. FEES

Fee/Charges to be paid to get the service : Fee prescribed by the organization

Maximum number of days to wait to get this service delivered :15 Working Days
