Is there any combined form of 8 and 8A? As my wife wants to change her surname & address altogether at one go. If not, then what is the process for the same?

Form 8 is filled up for minor corrections like the change of surname and if the new address lies in the same Part Number, both corrections can be done through Form 8. If the new address lies in the same AC but in a different Part Number, Form 8 A will have to be filled up for change in address.

Contact BLO officer and request him/her to reject it. Once rejected, you can apply for new application again.
