അപ്പ്ലിക്കന്റിന്റെ എസ്എസ്എൽസി സെര്ടിഫിക്കറ്റ് ഉണ്ടെങ്കിൽ അത് അപ്ലോഡ് ചെയ്യുക. മാതാവിന് എസ്എസ്എൽസി സെര്ടിഫിക്കറ്റ് ഉണ്ടെങ്കിൽ അത് അപ്‌ലോഡ് ചെയ്യുക.

ഫാദറിന് എസ്എസ്എൽസി സെര്ടിഫിക്കറ്റ് ഇല്ലാത്തതിനാൽ ഒരു അഫിഡവിറ്റ്‌ നൽകുക. അഫിഡവിറ്റ് വില്ലജ് ഓഫീസർക് അഡ്രസ്സ് ചെയ്യുക. അതിനുശേഷം നമ്മുടെ അഡ്രസ് ഇന്നെയാളാണ് അഫിഡവിറ്റ്‌ സബ്മിറ്റ് ചെയുന്നതെന്ന് എന്ന് പറയുക. തുടർന്ന് താഴെ രേഖപ്പെടുത്തുക അച്ഛൻ പേര് പറഞ്ഞിട്ട് ഇന്നയാൾ തലമുറകളായി ഇന്ന മതത്തിൽ ഇന്ന ജാതിയിൽ ജനിച്ചു വളർന്ന ജീവിച്ചുവരുന്ന ആണ് എന്നും നിലവിലും ആ മതത്തിൻറെ ആ ജാതിയുടെ ആചാരാനുഷ്ഠാനങ്ങൾ പിന്തുടർന്നു ജീവിച്ചു വരികയാണെന്ന് ബോധിപ്പിച്ചു കൊള്ളുന്നു എന്ന് രേഖപ്പെടുത്തി താഴെ ഒപ്പിട്ടു കൊണ്ട് അഫിഡവിറ്റ് ആയിട്ട് കൊടുക്കാവുന്നതാണ്
