I missplaced my DL and did not remember my DL number. Can I get my driving license number based on my DOB and Address ?

Visit the RTO from where you got your license and provide the below details:

• Your's and your father name
• Your Date of Birth

The details can be fetched in their system with these details. Ask them for DL-Extract for which they charge Rs. 20/-. After this you have to get the following documents:

• Police complaint (FIR copy)
• Affidavit (notary)
• Your age proof and address proof
• Post cover (DL will be dispatched through speed post)
• Pay duplicate DL fees (Rs. 215/-) and obtain acknowledgement

You will receive your DL in 30 days through Indian postal services.

Things to do if you lost your driving licence:

• Go to the police station that has jurisdiction over the area where you lost your driving licence.

• Lodge a complaint and make sure you get a copy of the First Information Report (FIR).

• Visit the Notary office to get an affidavit on a stamped paper. The affidavit will act as a proof that you took an oath before the concerned officer (executive magistrate/notary public/first class judicial magistrate) that your license is lost.

• You will have to pay a small fee for the notary service and the stamped paper.

Documents to be submitted for duplicate driving license

Submit the following documents at the RTO which issues your driving license.

• An FIR referring to the loss of the license

• Application Form LLD (for intimation of loss or destruction of driving license and application for duplicate)

• Details of the driving license

• Passport-sized photographs

• Address proof documents

• Age proof documents 

All the details in these forms will be cross-checked with the information already in the systems to get the duplicate license.
