1. ಎಸ್.ಎಸ್.ಎಲ್.ಸಿ ಪರೀಕ್ಷೆಯ ನಕಲು (ದ್ವಿತೀಯ, ತೃತೀಯ ಮತ್ತು ನಾಲ್ಕನೇ ಪ್ರತಿ ಅಂಕಪಟ್ಟಿ ಮತ್ತು ಅನುತ್ತೀರ್ಣ ಅಂಕಪಟ್ಟಿ ಪಡೆಯಲು ಇಚ್ಚಿಸುವ ಅಭ್ಯರ್ಥಿಗಳು ಈ ಮೂಲಕ ಆನ್ಲೈನ್ನಲ್ಲಿ ಅರ್ಜಿ ಸಲ್ಲಿಸಬಹುದಾಗಿದೆ. 
Candidates who wish to get S.S.L.C Duplicate Marks Card (Second, Third and Fourth Marks Copy of Marks Card) and Fail Marks Card can apply through online here. 
2. ಎಸ್.ಎಸ್.ಎಲ್.ಸಿ ಅಂಕಪಟ್ಟಿಯ ದ್ವಿತೀಯ, ತೃತೀಯ ಮತ್ತು ನಾಲ್ಕನೇ ಪ್ರತಿಯ ಅಂಕಪಟ್ಟಿ ಪಡೆಯಲು ಆನ್ಲೈನ್ ಮೂಲಕ ಅರ್ಜಿ ಸಲ್ಲಿಸುವ ಮುನ್ನ ಈ ಕೆಳಕಂಡ ದಾಖಲೆಗಳನ್ನು 500KB ಇಂದ 01MBರೊಳಗೆ pdf ಮಾದರಿಯಲ್ಲಿ ಸ್ಕ್ಯಾನ್ ಮಾಡಿಟ್ಟುಕೊಂಡು ಅಪ್ಲೋಡ್ ಮಾಡುವುದು. 
While applying for SSLC Duplicate Marks Card (Second, Third and Fourth Copy) through online the following documents must be scanned and should be within 500kb to 1mb in PDF format and uploaded. 
a) ಅಭ್ಯರ್ಥಿಯು “ಕಾರ್ಯದರ್ಶಿಗಳು, ಕರ್ನಾಟಕ ಶಾಲಾ ಶಿಕ್ಷಣ ಮತ್ತು ಮೌಲ್ಯನಿರ್ಣಯ ಮಂಡಳಿ, ………………………… ವಿಭಾಗ” ರವರಿಗೆ ಬರೆದಿರುವ ಅರ್ಜಿ. (ನಮೂನೆಗಾಗಿ ಇಲ್ಲಿ ಕ್ಲಿಕ್ಕಿಸಿ) 
The Application sent by applicant must be addressedto “Secretary,Karnataka School Examination & Assessment Board …………Division”

b) ಅಫಿಡವಿಟ್ ಪ್ರತಿ . (ನಮೂನೆಗಾಗಿ ಇಲ್ಲಿ ಕ್ಲಿಕ್ಕಿಸಿ)

c) ಆಧಾರ್ ಕಾರ್ಡ್ ನ ಪ್ರತಿ 
Copy of Aadhar Card 
d) ಮೂಲ ಅಂಕಪಟ್ಟಿಯ ಪ್ರತಿ ಇದ್ದಲ್ಲಿ 
Copy of Original Marks Card if he/she is having the same 
3. ಎಸ್.ಎಸ್.ಎಲ್.ಸಿ ಅನುತ್ತೀರ್ಣ ಅಂಕಪಟ್ಟಿ ಪಡೆಯಲು ಇಚ್ಚಿಸುವವರು ಆನ್ ಲೈನ್ ಮೂಲಕ ಅರ್ಜಿ ಸಲ್ಲಿಸುವ ಮುನ್ನ ಈ ಕೆಳಕಂಡ ದಾಖಲೆಗಳನ್ನು 500KB ಇಂದ 01MBರೊಳಗೆ pdf ಮಾದರಿಯಲ್ಲಿ ಸ್ಕ್ಯಾನ್ ಮಾಡಿಟ್ಟುಕೊಂಡು ಅಪ್ ಲೋಡ್ ಮಾಡುವುದು. 
While applying for SSLC Fail Marks Cardthrough online the following documents must be scanned which must be within 500kb to 1mb in PDF format and uploaded. 
a) ಅಭ್ಯರ್ಥಿಯು “ಕಾರ್ಯದರ್ಶಿಗಳು, ಕರ್ನಾಟಕ ಶಾಲಾ ಶಿಕ್ಷಣ ಮತ್ತು ಮೌಲ್ಯನಿರ್ಣಯ ಮಂಡಳಿ, ………………………… ವಿಭಾಗ” ರವರಿಗೆ ಬರೆದಿರುವ ಅರ್ಜಿ (ನಮೂನೆಗಾಗಿ ಇಲ್ಲಿ ಕ್ಲಿಕ್ಕಿಸಿ) 
The request letter sent by applicant must be addressed to “Secretary, Karnataka School Examination & Assessment Board, ..........Division”

b) ಆಧಾರ್ ಕಾರ್ಡ್ ನ ಪ್ರತಿ

4. ನಿಗದಿತ ಶುಲ್ಕ ಪಾವತಿಯ ನಂತರವೂ, Payment status not paid ಎಂದು ಬರುತ್ತಿದ್ದಲ್ಲಿ, ಶುಲ್ಕ ಸಂದಾಯ ಮಾಡಿರುವ – PGI reference ಸಂಖ್ಯೆ , ಅರ್ಜಿ ಸಂಖ್ಯೆ, ಅಭ್ಯರ್ಥಿಯ ಹೆಸರು ವಿವರಗಳನ್ನು ಮುಂದಿನ ಕ್ರಮಕ್ಕಾಗಿ 
sadpi-cc-kseeb@ka.gov.in ಗೆ ಮೇಲ್ ಮಾಡುವುದು. 
Even after the payment of prescribed fee, if the Payment status not paid, then the details of fee paid – PGI reference number, Application number, Candidate name has to be mailed to sadpi-cc-kseeb@ka.gov.in for further follow up at this end. 
ಈ ಮೇಲಿನ ದಾಖಲೆಗಳನ್ನು ಅಪ್ಲೋಡ್ ಮಾಡಿ ನಿಗದಿತ ಶುಲ್ಕ ಪಾವತಿಸಿದ ನಂತರ ಸಂಬಂಧಿಸಿದ ವಿಭಾಗೀಯ ಕಚೇರಿಗಳಲ್ಲಿ ಪ್ರಕ್ರಿಯೆಗೊಳಿಸಿ ಅಂಕಪಟ್ಟಿಗಳನ್ನು ಅಭ್ಯರ್ಥಿಯ ಆಧಾರ್ ಕಾರ್ಡ್ನಲ್ಲಿ ನಮೂದಾಗಿರುವ ವಿಳಾಸಕ್ಕೆ ರವಾನಿಸಲಾಗುವುದು. 
After uploading the above documents, payment of prescribed fee the application will be processed in concerned divisional office and Marks card will be sent through registered post to address mentioned in the candidate’s Aadhar card.
