I have two member ID which is also showing in the passbook section in mentioned UAN no. But only one ID is showing at the time of claim, where I am currently an employee. So I can't claim the first form.

Withdrawal cannot be done directly from previous PF account nos. They need to be transferred online to the new PF account no. Then withdraw from the new one.
