I can't understand your question because you didn't explained why did you changed your name i mean for what purpose you had changed your previous name, if this is for passport purpose you need to make atleast two documents in new name, the simple solution is, if you have your old name in your aadhar you need to change the name in aadhar by providing a Gazzetted verification letter. and the second document is you may apply for Pan or if you have a bank account you can change thename on it by simply submitting a covering letter along with vopy of name change affidavit and news paper cutting with newly changed aadhar.
