സ്വന്തമായിട്ടുള്ള അഫിഡവിറ്റിൽ കാണിക്കേണ്ടത് പരമ്പരാഗതം ആയിട്ട് കുടുംബപരമായിട്ട് ആയിട്ട് തലമുറകൾ ആയിട്ട് ഈ ലാറ്റിൻ കത്തോലിക്ക വിഭാഗത്തിൽ ജനിച്ചു ജീവിച്ചു വളർന്നുവരുന്ന ആളാണ് എന്ന് കാണിക്കുന്ന ഒരു അഫിഡവിറ്റ് വില്ലജ് ഓഫീസർ മുൻപാകെ എഴുതി സമർപ്പിച്ചു വില്ലജ് ഓഫീസറുടെ അറ്റസ്റ്റേഷൻ സീൽ വെച് ഓൺലൈൻ അപ്പ്ലിക്കേഷനോട് കൂടി അപ്‌ലോഡ് ചെയ്യണം.

അതോടൊപ്പം തന്നെ മാതാപിതാക്കളുടെ മതം സൂചിപ്പിക്കുന്ന 2 അയൽ സാക്ഷികളുടെ സ്റ്റെമെന്റ്റ് വെള്ള പേപ്പറിൽ രേഘപെടുത്തിയിട്ട് അയൽ സാക്ഷികളുടെ ആധാർ കാർഡിന്റെ കോപ്പി സഹിതം വില്ലജ് ഓഫീസർ മുൻപാകെ ഹാജരാക്കി അത് അറ്റസ്റ്റ് ചെയ്ത സീൽ ചെയ്ത് ഓൺലൈൻ അപ്പ്ലിക്കേഷനോട് കൂടി അപ്‌ലോഡ് ചെയ്യണം.

Churchil നിന്ന് ഒരു കത്ത് കൂടി ഉണ്ടെന്ന് പറഞ്ഞു. ഒന്നുകൂടി വിശാലമായ ഒരു കത്തു കൂടി മതസംഘടനയുടെ ഹാജരാകാൻ കഴിയുമെങ്കിൽ അതും കൂടി അപ്ലോഡ് ചെയ്തു വീണ്ടും ഒരു ആപ്ലിക്കേഷൻ കൊടുക്കുക.

തഹസിൽദാറിൽ നിന്ന് ഈ സർട്ടിഫിക്കറ്റ് ലഭിക്കുന്നില്ല എങ്കിൽ ഒരു അപ്പീൽ എന്ന നിലയിൽ മാന്വൽ ആയിട്ട് ഒരു അപ്ലിക്കേഷൻ തയാറാക്കി ഓൺലൈൻ ആയിട്ടുള്ള അപ്ലിക്കേഷൻ റീജെക്ട ചെയ്തു എന്നും രണ്ടാമത് വീണ്ടും അപ്ലൈ ചെയ്‌തു വീണ്ടും റീജെക്ട ചെയ്തു എന്ന് കാണിച്ചു കൊണ്ട് ഒരു അപ്പീൽ എന്ന നിലയിൽ ആർ ഡി ഓ മുൻപാകെ ഒരു അപേക്ഷ കൊടുക്കുക.
