Whom to approach for this service (Designated Officer)? Committee headed Deputy Commissioner
Procedure involved to get this service Applicants who belong to Scheduled Caste / Schedule Tribe can submit the prescribed application along with necessary documents to District Social Welfare Officer. Applicants who belong other Backward classes, can submit the prescribed application along with necessary documents to District Officer, Other Backward Class and Minority Welfare Department.
Who are eligible to get this service Citizens in need of this service
Documents to be enclosed with the request

 

1 .  If any of the parents are employees, caste as per service register

2 . Caste mentioned in following documents i. Primary school certificate ii. Transfer certificate iii. Cumulative record

3 . If the applicants brother/sister studying their school/ college certificate.

4 . Caste certificate issued by tahasildar

5 . Caste certificates of parents

6 . If parents are literates, school certificates of the parents such as t.c/ cumulative record

7 . Affidavit on rs.20/- bond paper

8 . Pass port size photo-3

9 . Election voters id card

10. Family tree

11 . Residential certificate issued from revenue officers

12 . Attested copies of above documents to be submitted.
