Procedure involved to get this service: 

1. Prescribed applications should be submitted with details like date of Death, Name.

2. Receipt is issued for payment

3. Based on the details it will be searched

4. Certificate will be printed and issues to the applicant

Form to be submitted to get this service: Prescribed Death Certificate application which will be supplied free at the counter.

Who are eligible to get this service: Citizen requesting for certificate where the death has taken place under BBMP jurisdiction

1 . DOCTOR CERTIFICATE
2 . FORM 2
3 . ADDRESS PROOF
4 . CRIMINATION REPORT
