3. IDENTITY CARD ISSUED BY THE EDUCATIONAL INSTITUTION IN CURRENT YEAR (OPTIONAL FOR PRIMARY AND HIGH SCHOOL STUDENTS)
4. RECENT PHOTOGRAPH OF THE STUDENT.
