Here is the step-by-step procedure to get proof of address affidavit.
Step 1: Prepare an affidavit draft in word format. A draft looks like a below image.
Step 2: Buy e-stamp paper from co-operative bank, court, or registrar office near you.
The stamp duty for an affidavit ranges from Rs. 20 -100/-
Details require to buy an e-stamp paper.

Stamp duty amount (Rs. 20/ 50 /100)
An e-stamp paper looks like the below image.
Step 3: Print the affidavit draft on an A4 size paper.
Step 4: Carry the printed affidavit and supporting documents to the notary near you.
A supporting document could be any government approved ID proof for notary verification (Aadhar, Passport, or driving license)
Step 5: Sign the Affidavit in front of a notary. A notary seals and sign an affidavit and make an entry in notary book.
This completes the procedure of affidavit execution for address proof.
