ജനന രജിസ്റ്ററിലെ പേരിലുള്ള മൈനർ കറക്ഷൻസ് സ്കൂൾ രേഖയുടെ പകർപ്പുസഹിതം അപേക്ഷിച്ചാൽ അനുവദിക്കുന്നതാണ്.

എന്നാൽ  സ്പെല്ലിങ്ങിലുള്ള വ്യത്യാസം മൂലം പേര് മറ്റൊന്നായി മാറുന്ന സാഹചര്യം ഉണ്ടെങ്കിൽ ചുവടെ പറയുന്നവ സഹിതം  അപേക്ഷിക്കണം 
 
1) അപേക്ഷകന്റെ ഫോട്ടോ പതിച്ച തിരിച്ചറിയൽ രേഖ.
 
2) മുൻപ് ജനന സർട്ടിഫിക്കറ്റുകൾ വാങ്ങിയിട്ടുണ്ടെങ്കിൽ അവ. (വ്യത്യസ്തങ്ങളായ സർട്ടിഫിക്കറ്റുകൾ കൈവശം വയ്ക്കുന്നത് തടയുന്നതിനാണ് ഈ നിബന്ധന)
 
3) വാങ്ങിയ സർട്ടിഫിക്കറ്റുകൾ നഷ്ട്പെട്ടിട്ടുണ്ടെങ്കിൽ അതുസംബന്ധിച്ച 200 രൂപയുടെ  മുദ്രപത്രത്തിലുള്ള  സത്യവാങ്മൂലം.
 
4) ജനന രജിസ്റ്ററിൽ ഒരിക്കൽ ചേർത്ത പേര് തിരുത്തുന്നതിനുള്ള സാഹചര്യം വ്യക്തമാക്കുന്ന 200 രൂപയുടെ  മുദ്രപത്രത്തിലുള്ള സത്യവാങ്മൂലം 
 
5) കോമ്പൗണ്ടിങ് ഫീസ് 50 രൂപ.
 
6) പുതിയ സർട്ടിഫിക്കറ്റിനുള്ള 50 രൂപയുടെ മുദ്രപത്രവും സർട്ടിഫിക്കറ്റ് ഫീസായി 5 രൂപയും കൂടി നൽകണം.
 
7) രണ്ടു പേരിലും ഉള്ളയാൾ ഒരാൾ തന്നെയാണെന്ന് തെളിയിക്കുന്നതിനുള്ള രേഖകൾ  
 
ഗസറ്റ് വിജ്ഞാപന പ്രകാരം താങ്കൾ പുതിയ പേര് സ്വീകരിച്ചാൽ ജനന രജിസ്റ്ററിൽ നിലവിലുള്ള പേരിനൊപ്പം പുതിയ പേര് alias ചേർത്തു് സർട്ടിഫിക്കറ്റ് നൽകും.
 
ഉദാഹരണം: Rekha alias Lekha (രേഖ ജനന രജിസ്റ്ററിലെ നിലവിലുള്ള പേരും, ലേഖ ഗസറ്റ് വിജ്ഞാപന പ്രകാരം മാറ്റിയ പേരും ആണെങ്കിൽ).
